Office hours and scheduling

We require a referral from your physician and a copy of your medical records prior to scheduling a new patient appointment. See Release of Medical Records

New Patient Consultations

New patient consultations include not just the visit, but also chart review and preparation prior to your visit. New patient visits usually last 90 minutes. Consults for symptom management require an in-person initial visit.

Follow-up visits

Follow-up visits are scheduled every month until there is improvement in symptoms. If schedule II medications are prescribed, monthly visits must occur per federal regulations.

Phone calls, portal messaging, prior authorizations, documentation, disability forms, letters, revisions, refills, and other services requiring more than 5 minutes are billed $30.
